Contact your insurance provider and request a summary of benefits and coverage (SBC) related to physical therapy. That document will typically outline what is covered and what you can expect to pay under your plan.

We accept all forms of credit cards, debit cards and personal checks for payment for visit fees, co-pays or co-insurance – whatever is appropriate based on your insurance plan. We are able to contact your insurance to confirm and review your physical therapy benefits for you.
